Caring Choices HIV / AIDS Assistance

    Caring Choices has been providing HIV/AIDS services throughout the north state since 1993. It offers programs that will help serve individuals with HIV/AIDS and their families. Staff includes Registered Nurses and Mastered Prepared Social Work Case Managers who have in-depth experience to assess and evaluate which specific programs will meet the individual needs of those with HIV/AIDS.

    Programs

    AIDS Drug Assistance Program (ADAP): This program allows for eligible individuals to obtain their HIV/AIDS medications at no cost. Covered drugs must have been proven to improve the quality of life or prevent and treat HIV/AIDS diseases.

    Care Model Program: This program provides support for critical HIV/AIDS services. Services under this program include:

    • Medical Case Management (please refer below for more info)
    • Emergency Financial Assistance
    • Medical Transportation
    • Oral Health Care
    • Out-patient Ambulatory Medical Care

    HIV/AIDS Housing Assistance: The Housing Opportunities for Persons with AIDS (HOPWA) Program is funded through the U.S. Department of Housing and Urban Development (HUD). The California Department of Public Health (CDPH), Center for Infectious Diseases, Office of AIDS (OA) assumes the grant administration responsibilities, as the State of California HOPWA grantee.

    The program is designed to provide resources, incentives, and strategies to meet the various housing needs of persons living with HIV/AIDS (PLWH/A) who are homeless or at risk of becoming homeless. The goal of the HOPWA Program is to assist clients in maintaining housing stability and to improve their access to health care and supportive services. OA works toward these goals by funding local government agencies and community-based nonprofit organizations to undertake HOPWA-eligible activities to meet the most urgent needs not being met by other available public and private resources.

    OA allocates the State of California (HOPWA) grant on an annual basis to 40 counties that are not eligible to receive HOPWA funding directly from HUD. Funds are allocated to those counties through a non-competitive formula allocation process. Caring Choices is the HOPWA provider of services for Butte, Glenn, Tehama, Shasta, Trinity, Yuba, Sutter, and Colusa counties.

    Housing assistance and related supportive services to HIV/AIDS individuals including:

    • Rental assistance
    • Short-term payments to prevent homelessness
    • Assistance with utilities (gas, electric, wood, propane)

    HIV/AIDS Food Closet: Food items are made available to people living with HIV/AIDS and their families. Food is made available to be distributed during our support group meetings and in some cases our nurses and social workers will drop off food bags when making house visits to their clients.

    Medical Case Management Services

    Case Management by a Registered Nurse: Comprehensive nursing case management services are provided on an ongoing basis to assist with the complexities of the HIV/AIDS disease process.

    • Face to face nursing assessments
    • Monitor medical needs
    • Provide nutritional counseling
    • Medication teaching
    • Provide information about diagnosis
    • Physician Referrals
    • Answer questions
    • RN advice nurse available 24 hours per day

    Case Management by a Social Worker: Comprehensive social work case management services are provided on an ongoing basis to assist with the social, financial and emotional aspects of an individual with HIV/AIDS.

    • Face to face psychosocial and financial assessments
    • Benefits counseling
    • Advocacy with Social Security, Disability and Social Services
    • Budget Planning
    • Assistance with legal issues/housing
    • Referrals to counseling, community resources and Support Groups
    • Assistance with medical transportation and food

    Referrals: Our staff can make referrals to agencies and professionals for Supplemental Services which include Support Group Meetings, Individual Counseling Sessions, and Health Insurance Premium Payment Program (HIPP). Our Mission

Caring Choices provides innovative, low or no cost health and wellness services to seniors, disabled adults, families affected by cancer, and persons living with HIV/AIDS.

The staff of nurses and case managers plays a pivotal role in identifying and filling the region’s special needs. Caring Choices’ mission of “…providing services that are currently non-existent” has resulted in staff developing and implementing a wide range of unique programs to address the area’s unmet needs.
